Name Badge Order Form

Badge Example Name badges for faculty and staff can be ordered online. Administrative assistants who would like to order a name badge for employees in their department should submit the following form.

Name badges are printed by Bomar Trophy Shop in Lakeland. The cost is $11.50 each (one to 14 name badges). Bulk orders (more than 15) are $10.50 per name badge. Shipping prices begin at $7 per order. Name badges will be printed according to the exact information provided in the form submission. Invoices will arrive in the shipment and can be paid with a credit card by calling 863-682-1439. Please allow two weeks for delivery. For more information send an email to Marketing.

Only those authorized to order name badges for their respective departments may submit this form and must use official department names as shown on the University's organizational chart.
