All incoming students must submit a medical health history form, including record of mandatory immunizations, completed and signed by your doctor. All immunizations (including PPD/TB Test) on the Mandatory Immunization and Health Form below are required for this form to be complete. Getting an appointment with your physician can take time, so please schedule your physical as soon as possible.
